PBF Unitholder Verification Form & Requirements

To all our valued Unit Holders/Shareholders of the Pacific Balance Fund.

The following are the requirements for lodgements made:



For Individual Unit Holders:

  • Complete PBF Unitholder Verification form

  • Formal Letter addressed to Management for application lodged.

  • PBF Certificate

  • 2x Valid ID

  • 6 months Bank Statement

  • State Declaration Form

For transfers of Units from deceased accounts:

  • Complete Unitholders verification form

  • Complete Unit Holder update form for Next of Kin Information

  • Formal Letter addressed to Management for application lodged.

  • Death Certificate

  • PBF Certificate

  • 2x Valid ID for both deceased and next of kin.

  • 6 months Bank statement

  • State Declaration form

  • Public Curators Authority letter of Share Transfers
